In order to improve the transition from elementary school to middle school, we are looking for feedback from fifth grade students and their parents, as well as  fourth grade students and their parents.  Below you will find a web link for an online survey. We are asking fourth and fifth grade students along with a parent to complete the survey by Tuesday, March 29, 2011. The survey should take about 10 minutes to complete, it consists of 10 questions, and can be completed on any  computer with internet access. Should you not have internet access, please contact Amy Coleman at Elk Valley Elementary to receive a paper copy of the survey, or to make other arrangements to complete the survey.
